Guidelines for Designing Data Collecting Evaluation Tools

  1. Determine the type of your Extension training and the audience.
  2. Select the appropriate evaluation tool template for your Extension training and the audience.
  3. Modify the selected evaluation tool template to match your program content. If you need help at this step, you may review a sample tool in the selected tool category.
  4. Complete the needed changes of the tool to match with your Extension training.
  5. Save the modified evaluation tool and print copies needed for your training.
  6. Use the tool for collecting data.
  7. If you intend to collect data online, select the appropriate evaluation tool template and build the survey on Qualtrics.
